When You Will Receive Your Form 1095-C

If you request a copy of your Form 1095-C, TCG US AcquistionCo, LLC will provide it no later
than:

• March 2 of the year following the calendar year the form covers, or
• 30 days after the date we receive your request, whichever is later.
